LIC NEFT Form Documents Required:

• Canceled check leaf with account number and name printed (or) Cancelled checks leaf together with a copy of the page in the bank passbook that contains the details of account holders if the name and number are not printed on the canceled cheque leaf.

NEFT option required for the holders/claimants

• Please send the NEFT application to our branch or customer zone. 
• The policyholder/claimant will have to submit his unsigned check sheet or a photocopy of the passbook/checkbook containing complete details of his account. 
• Please contact your branch if you have not received a deposit on your account two days before the due date. 
• The account of the annuitant or policyholder will only be used at the time the policy is paid.
