- A passport-size photograph

- Form 8 – For correction of mistakes, change of address, replacement of card, marking of differently-abled
How to Add Name to Voter List Online
- Visit the Election Commission website.
- Click on the FORMS link.
- Click on " Form 6 Registered as a New Voter/Elector ".
- Enter the details and upload the required documents.
- Click on "Preview and Submit".
